Summary
Illinois Medicaid hospice providers must now use a new mandatory form (HFS 1592) to notify the state of hospice benefit elections, patient deaths, and transfers. The policy also reinforces requirements for timely physician certifications and background checks for hospice staff.
Action Required
By May 1, 2014: Hospice billing teams must begin using the mandatory HFS 1592 form for all Illinois Medicaid hospice elections, deaths, transfers, and care cessations. Download the PDF-fillable form from the Medical Programs Forms Page. Submit forms to both HFS and managed care entities (MCEs) when applicable. Allow two weeks after HFS 1592 submission before filing claims. Retain all Notice of Election forms for HFS audits.
