Summary
Long Term Care (LTC) case management is transitioning from local Family Community Resource Centers to centralized hubs in Chicago and Decatur. Providers must update their contact information and send all LTC correspondence to the appropriate regional hub based on their location.
Action Required
By December 1, 2014 (Region 2) and January 1, 2015 (Region 3): Billing and administrative staff must update contact records to send all LTC correspondence to the appropriate centralized hub - MFO in Chicago for Region 2 cases or Macon LTC Office for Region 3/5 cases. Update billing system contact information and provider manuals with new addresses and phone numbers. Failure to use correct contacts may delay case processing and reimbursements.
