Summary
Illinois hospitals discharging Medicare-eligible patients to skilled nursing facilities must now provide discharge planners with copies of Care Coordination Unit assessments and accompanying materials. Discharge planners are responsible for transmitting these documents when patients are transferred to skilled nursing facilities.
Action Required
Immediately: Hospital discharge planning staff must ensure they receive copies of CCU prescreening assessments and accompanying materials when Medicare patients are being discharged to skilled nursing facilities.
