Long-term care facilities only: Email facility name, designated contact person, phone number, and facility-specific email address to HFS.LTC@illinois.gov with subject line 'Redetermination List Contact' to receive monthly notifications of residents scheduled for Medicaid eligibility redetermination. Must immediately notify HFS of any changes to designated representative or contact information.
