By October 1, 2015: Long-term care facilities must obtain ICD-10 codes from physicians/clinicians and update all existing resident ICD-9 codes to ICD-10 codes using the MEDI system. For new admissions from July 1 - September 30, 2015, enter both ICD-9 and ICD-10 codes. For admissions on/after October 1, 2015, only ICD-10 codes are required. Facilities using EDI vendors must register with MEDI for this one-time conversion.
