Summary
Laboratory and portable X-ray providers must use ICD-10-CM diagnosis codes instead of ICD-9 codes on HFS 2211 claim forms effective October 1, 2015. Providers must also use the revised claim form with revision date R-2-15 for all services provided on or after this date.
Action Required
By October 1, 2015: Laboratory and portable X-ray providers must order revised HFS 2211 claim forms (revision date R-2-15) from the Medical Forms Request page and update billing systems to use ICD-10-CM diagnosis codes in Item 24 instead of ICD-9 codes. Update Handbook Appendix L-1 by replacing pages 1-6 with new version from department website. Claims with dates of service on/after October 1, 2015 submitted on old forms or with ICD-9 codes will not be processed.
