Summary

Home health agencies must transition from ICD-9 to ICD-10-CM diagnosis codes for all services provided on or after October 1, 2015. A revised HFS 2212 claim form (revision R-2-15) must be used for billing these services, and the provider handbook has been updated accordingly.

By October 1, 2015: Home health agencies must order and use the revised HFS 2212 claim form (revision R-2-15) from the Medical Forms Request page. Update billing systems to use ICD-10-CM diagnosis codes instead of ICD-9 for all dates of service on or after October 1, 2015. Replace Appendix R-1 pages in the provider handbook with updated version. Claims submitted with old forms or ICD-9 codes after the effective date may be rejected.
