Summary
Illinois Medicaid has reissued the Handbook for Providers of Medical Equipment and Supplies (Chapter M-200) with updated ICD-10-CM diagnosis coding requirements and revised claim forms. Providers must use the new HFS 2210 form (revision R-2-15) for all durable medical equipment claims with dates of service on or after October 1, 2015.
Action Required
By October 1, 2015: Billing team must obtain and implement the revised HFS 2210 claim form (revision R-2-15) from the Medical Forms Request page for all durable medical equipment billing. Review updated Appendices M-1 and M-2 for proper claim preparation procedures. Ensure all ICD-10-CM diagnosis coding complies with expanded requirements for dates of service on or after October 1, 2015.
