For partial hours: bill in 15-minute increments (9-22 min = 1/4 rate, 23-37 min = 1/2 rate, 38-52 min = 3/4 rate, 53-60 min = full rate). Submit replacement claims using Claim Frequency '7' for any incorrect payments received.
