By June 1, 2016: Billing team must update renal dialysis billing to include separate service lines for each treatment date, ensure Service Line Date (FL 45) is populated for revenue codes 0634, 0635, and 0636, and verify Epogen units in FL 46 equal 1/100 of Value Code 68 units in FL 39. Update billing software to prevent series claims with multiple dates on one line. Claims missing these requirements will be rejected with error codes E34, K42, E48, or C76.
