Summary
Illinois Department of Healthcare and Family Services has released the 2016 Long Term Care cost report forms and instructions. LTC facilities must submit their cost reports using the new 2016 Excel format by September 30, 2016, or 90 days after their fiscal year end, whichever is later.
Action Required
By September 30, 2016: Long term care facilities in Illinois must download the 2016 LTC cost report Excel file from the Illinois Department of Healthcare website, complete it using only the 2016 format, and submit electronically to HFS.HealthFinance@illinois.gov plus mail a signed paper copy to Bureau of Health Finance in Springfield. Previous year report forms will not be accepted.
