Providers must document medical necessity using the 18 specified criteria (age ≤18, surgery >3 hours, BMI ≥40, pregnancy, ASA III+, anesthesia complications history, difficult airway, moderate/severe OSA, uncontrolled asthma/COPD, ventilator dependence, advanced liver disease, uncontrolled diabetes, ESRD on dialysis, recent MI, ongoing cardiac ischemia). Update scheduling workflows to verify appropriate site of service before procedure booking. Claims may be denied without proper authorization.
