Summary
BCBSAZ now requires prior authorization review for outpatient procedures performed in hospital outpatient departments when they could be safely performed in ambulatory surgery centers or physician offices. Hospital outpatient settings will only be approved when specific medical criteria are met, such as ASA III+ classification, morbid obesity, or complex medical conditions.
Action Required
By August 1, 2026: Billing team must implement prior authorization requests for all outpatient procedures scheduled in hospital outpatient departments. Providers must document medical necessity using the 18 specified criteria (age ≤18, surgery >3 hours, BMI ≥40, pregnancy, ASA III+, anesthesia complications history, difficult airway, moderate/severe OSA, uncontrolled asthma/COPD, ventilator dependence, advanced liver disease, uncontrolled diabetes, ESRD on dialysis, recent MI, ongoing cardiac ischemia). Update scheduling workflows to verify appropriate site of service before procedure booking. Claims may be denied without proper authorization.
