Summary
Illinois Department of Healthcare and Family Services updated Medicaid hospital cost report forms and instructions. All cost reporting hospitals must use the new Excel format available on the Department's Cost Report webpage, as previous versions will no longer be accepted.
Action Required
Immediately: Hospitals must download the updated Excel Medicaid cost report forms from the Illinois Department of Healthcare and Family Services Cost Report webpage under Hospital Providers section. Submit completed reports within five months after fiscal year close via CD (Excel format for Medicaid, PDF for Medicare) plus signed paper copies to Bureau of Health Finance, 201 South Grand Avenue East, Springfield, IL 62763. Do not use previous cost report versions as they will not be accepted.
