Immediately: Long Term Care facilities must update contact information and workflow procedures to use the correct regional Medical Field Operations office based on facility county location. Update internal directories with new phone numbers, fax numbers, and email addresses for Region 1 (Cook County), Region 2 (16 northern counties), Region 3 (24 central counties), Region 4 (28 central/western counties), and Region 5 (32 southern counties). Route all DHS medical field operations inquiries to appropriate regional hub.
