Summary
The Illinois Department of Healthcare and Family Services has released the 2017 Long Term Care cost report forms and instructions for nursing facilities, ICF/IID facilities, and specialized mental health rehabilitation facilities. Cost reports must be submitted by September 30, 2017 or 90 days after the facility's fiscal year end, whichever is later.
Action Required
By September 30, 2017: Long term care facilities must download the 2017 Excel cost report form from the Illinois HFS Cost Report webpage and submit completed reports via email to HFS.HealthFinance@illinois.gov along with a signed paper copy mailed to Bureau of Health Finance in Springfield. Only use the 2017 forms - older printed versions will not be accepted.
