Summary
The Illinois Department of Human Services has identified a problem with the fax number on Medicaid Eligibility Redetermination (REDE) forms that prevents successful document submission. Providers must temporarily use alternative mailing addresses or working fax numbers for the three regional Medical Field Operations offices instead.
Action Required
Immediately: Long-term care providers and supportive living facilities must stop using the faulty fax number printed on REDE forms. Instead, submit all Medicaid eligibility redetermination forms and supporting documentation via mail or working fax numbers to the appropriate regional Medical Field Operations office (North, Central, or Downstate based on location). Update internal procedures to reference correct contact information until the form issue is resolved.
