Hospitals must submit completed reports within five months of fiscal year end using the new Excel format, with previous versions no longer accepted.

Within five months of hospital's fiscal year end: Download updated Excel Medicaid hospital cost report forms from Illinois HFS Cost Report webpage. Submit completed Medicaid report (Excel format) and Medicare report (PDF format) via CD, plus mail signed paper copies to Bureau of Health Finance at 201 South Grand Avenue East, Springfield, Illinois 62763.
