Summary
Illinois Department of Healthcare and Family Services has released the 2018 Long Term Care cost report forms and instructions. LTC facilities must use the new 2018 Excel forms (not older versions) and submit completed reports by September 30, 2018 or 90 days after fiscal year end, whichever is later.
Action Required
By September 30, 2018 (or 90 days after fiscal year end): Long term care facilities must download the 2018 Excel cost report forms from the Illinois HFS Cost Report webpage, complete using only the 2018 version, and submit via email to HFS.HealthFinance@illinois.gov plus mail signed paper copy to Bureau of Health Finance in Springfield. Do not use 2017 or earlier forms as they will not be accepted.
