Effective July 1, 2018: Billing team must add CDT code D1354 to covered procedures in dental billing system. Configure system to limit billing to maximum 4 teeth per day and enforce restrictions preventing same-day billing with D2000 (Restorative) or D3000 (Endodontic) codes on same tooth. Set annual limit of 2 applications per tooth with lifetime maximum of 6 applications per tooth. Update fee schedule from Illinois Department of Healthcare and Family Services website.
