Summary

Effective October 1, 2018, Illinois Medicaid (HFS) no longer allows multiple patients to be billed on the same HFS 2210 claim form for durable medical equipment and supplies. Each claim form must contain only one Recipient Identification Number and associated services. Claims with multiple patients will be returned for correction.
