Summary
Illinois requires a new form HFS 2270 (Physician Certification Statement) for all non-emergency transportation from hospitals or long-term care facilities, replacing the previous MCA and CTS forms. Action Required
By April 1, 2019: Healthcare facilities in Illinois must implement the new HFS 2270 (Physician Certification Statement) form for all non-emergency transportation requests. Medical staff must complete this form before patient transport and provide copies to transportation providers. Update discharge planning workflows to include PCS completion. Old MCA and CTS forms will no longer be accepted after April 1, 2019, which could result in transportation payment denials.
