Summary
Illinois Department of Healthcare and Family Services has updated Medicaid hospital cost report forms and instructions available on their Cost Report webpage. Hospitals must use the new Excel format and submit both Medicaid and Medicare cost reports within five months of fiscal year end, with no extensions unless CMS grants one for Medicare.

By five months after fiscal year end: Hospital billing teams must download updated Excel Medicaid cost report forms from Illinois HFS Cost Report webpage under Hospital Providers section. Submit completed Medicaid cost report (Excel format) and Medicare cost report (PDF format) via CD to Bureau of Health Finance at 201 South Grand Avenue East, Springfield, IL 62763. Also mail signed paper copies of both reports to same address. Do not use previous versions as they will not be accepted.
