Summary
Illinois Medicaid now requires prior approval for Bambini eyeglass frames available through Illinois Correctional Industries. These specialty frames will only be approved when health or behavioral conditions prevent the use of regular frames, and only apply to fee-for-service Medicaid patients (not managed care).

For Illinois Medicaid fee-for-service patients only: Before ordering Bambini frames, billing team must obtain prior approval by submitting HFS 1443 (Provider Invoice), HFS 2803 (Optical Prescription Order), and HFS 1409 (Prior Approval Request) with medical necessity documentation to Illinois Department of Healthcare and Family Services Optical Billing Consultant, P.O.
