Summary

Starting January 1, 2020, all Medicare claims must use the new Medicare Beneficiary Identifier (MBI) instead of the old Health Insurance Claim Number (HICN). Claims submitted without a valid MBI will be denied with error code K22.

By January 1, 2020: Billing team must update all systems to use Medicare Beneficiary Identifier (MBI) instead of Health Insurance Claim Number (HICN) for all Medicare claims. Update paper billing forms HFS 3797 and UB-04 formats. Ensure institutional claims include MBI in Form Locator 60. Claims without valid MBI will be denied with error code K22.
