Summary
Illinois Medicaid hospital cost reports and related Medicare cost reports must be submitted within five months after the hospital's fiscal year ends. Reports must use the updated Excel format available on the state website, with both electronic (CD) and signed paper copies required.
Action Required
Within five months of fiscal year end: Hospital billing teams must download updated Excel Medicaid cost report forms from Illinois Department of Healthcare and Family Services website. Submit completed Medicaid report (Excel format) and Medicare report (PDF format) via CD, plus mail signed paper copies to Bureau of Health Finance at 201 South Grand Avenue East, Springfield, IL 62763. Previous report versions will not be accepted.
