Summary
Transportation providers must now report loaded mileage in Loop 2400, Segment CR106 on electronic claim submissions for Medicaid fee-for-service, HealthChoice Illinois managed care, and MMAI plans. Claims without this required mileage information will be rejected with error code J74.
Action Required
Immediately (policy already effective): Transportation providers must update their electronic billing systems to report loaded mileage in Loop 2400, Segment CR106 on 837P claim format (example: CR1****A*DH*10). Notify clearinghouse or software company of required changes. Failure to include mileage will result in claim rejection with error code J74.
