If FQHC is facing immediate layoffs or closures: Submit request to HFS.BPRA@Illinois.gov with required financial documentation including Medicaid revenue from last 4 weeks by plan, number of empaneled patients by plan, weekly revenue for last 4 weeks through March 30-April 3, average monthly Medicaid revenue over past 6 months by plan, and number of days cash on hand. Only submit when advance is actually needed.
