Summary

Due to COVID-19, Illinois temporarily waived documentation requirements for non-emergency medical transportation from March 1, 2020 through the end of the public health emergency. The HFS 2270 Physician Certification Statement form and participant signatures are no longer required for prior approvals or claims.
