Immediately: All providers must ensure complete medical record documentation including progress notes, service plans, and signature logs for all Medicaid claims. Verify all attending and ordering/referring/prescribing providers have individual NPIs and are enrolled in IMPACT system. Review Chapter 100 of the general handbook for record requirements. Use the provider directory on IMPACT homepage to verify provider enrollment before submitting claims. Failure to comply may result in overpayment recovery, payment suspension, or provider termination.
