Submit any requests for rate review with revised cost information to Illinois Department of Healthcare and Family Services, Bureau of Program and Policy Coordination, 201 South Grand Avenue East, Springfield, Illinois 62763. No action needed for claim resubmission as HFS will automatically adjust previously filed claims.
