Illinois Medicaid now provides reimbursement for hospital long-term care days when patients cannot be discharged due to lack of available placement at a lower level of care. Hospitals must make five placement attempts and submit Form HFS 1329 to receive $289.48 per approved day.

For Illinois hospitals: When patients cannot be discharged due to lack of placement options, make five documented attempts to place patient in lower level of care, then complete and submit Form HFS 1329 via email to hfs.Hospitals@illinois.gov following HIPAA guidelines. Submit regular DRG or per diem claims identifying non-covered hospital long term care days through usual process. Only days after form submission will be reimbursed at $289.48 per day.
