Immediately: Ensure your practice has updated fax numbers on file with BCBSM/BCN or register for Direct Secure Messaging (DSM) through DirectTrust to receive notifications. Clinical staff must monitor for admission/discharge notices and document required follow-up actions in patient records: patient engagement within 30 days post-discharge and medication reconciliation within 31 days of discharge. Update workflows to access 1health EMR links provided in notices to close HEDIS gaps without manual medical record collection.
