Summary
Illinois Medicaid is implementing a new HealthChoice Illinois ADT platform to send electronic notifications to ambulatory providers when their Medicaid patients are admitted, discharged, or transferred from hospitals and skilled nursing facilities. Providers enrolled in the IMPACT system can subscribe to receive these HL7 formatted messages to improve care coordination.

By July 9, 2021: Complete the provider readiness survey at https://www.surveymonkey.com/r/DGJQDZB if your practice is enrolled in Illinois IMPACT system and provides care coordination for Medicaid patients. Consider attending demonstration sessions on June 24 or June 25, 2021. Prepare for onboarding beginning August/September 2021.
