Immediately: Billing team and patient accounts managers must establish workflow to report all third-party requests for Medicaid patient information to Illinois Department of Healthcare and Family Services. For Cook County residents, fax requests to 312-338-0304 or call 312-793-3528/312-793-3529. For all other counties, fax to 217-524-5672 or call 217-785-8711. Document patient name, DOB, RIN number, case number, service dates, and requesting entity information. Failure to report could indicate potential claims or liens for the Department.
