Due to sustained RSV prevalence, Illinois providers can now submit early dosing prior authorization requests for Synagis (palivizumab) through October 31, 2021, before the typical November 1st coverage period. This applies to both Medicaid fee-for-service and managed care plan customers.

By October 31, 2021: For Illinois Medicaid patients needing early Synagis dosing, complete the Drug Authorization Request Form and fax to 217-524-7264 for fee-for-service patients, or contact the appropriate managed care plan for approval guidance.
