Summary
The Illinois Department of Healthcare and Family Services is distributing final CARES Act funding grants totaling $45.9 million to Long-Term Care providers including nursing facilities, ICF/IIDs, supportive living programs, and MC/DD facilities. Eligible providers must sign grant agreements and submit completed budgets by December 1, 2021 to receive funding before it expires on December 31, 2021.

By December 1, 2021: Long-term care providers (NF, ICF/IID, SLP, MC/DD) must sign the Grant Agreement, complete the provided budget template, and return both documents to HFS in PDF (agreement) and Excel (budget) formats.
