This is a reminder that skilled nursing facilities, supportive living programs, and specialized mental health rehabilitation facilities enrolled in IMPACT with HL7-capable EHR systems must participate in HealthChoice Illinois ADT. Facilities not yet compliant must complete a progress survey by January 26, 2022.

By January 26, 2022: If your facility is a skilled nursing facility, supportive living program, or specialized mental health rehabilitation facility not yet compliant with HealthChoice Illinois ADT, complete the required survey at https://www.surveymonkey.com/r/6HQ9GQW to document onboarding progress.
