Summary

Starting February 1, 2022, Illinois reinstated the requirement for Physician Certification Statement (PCS) forms for all non-emergency ambulance and medical transports from hospitals and long-term care facilities. This applies to Medicaid fee-for-service, HealthChoice Illinois MCO plans, and MMAI plans, ending the temporary COVID-19 waiver.

Effective February 1, 2022: Billing team must ensure PCS forms (HFS 2270) are completed by hospital or LTC facility medical staff prior to all non-emergency ambulance transports. Verify forms are provided at time of transport - one form valid for 60 days for ambulance or 180 days for wheelchair van transports. Claims without proper PCS documentation may be denied for Medicaid patients.
