Before April 18, 2022: Staff involved in SMHRF and SLP screening processes must contact ILPASRR@maximus.com with subject line 'Please add me to the Illinois SMHRF and SLP contact list' to receive training information. Include full name, title, facility name and email address. Complete required training from Maximus to access the new AssessmentPro system for all future screening submissions.
