By May 1, 2022: Billing team must ensure HFS 2271 CTS forms are completed by qualified medical professionals for all non-emergency transports (ambulance, medicar/wheelchair van, service car) originating from patients' private residences. Provide copies to transportation providers at time of transport. Continue using HFS 2270 forms for transports from long-term care facilities or hospitals. CTS forms are valid for 60 days for ambulance transports and 180 days for van/car transports.
