Summary
Illinois has delayed the implementation of the Certificate of Transportation Services (CTS) form HFS 2271 until June 1, 2022. This form will be required for prior and post approval of all non-emergency medical transports originating from a customer's private residence under Medicaid FFS, HealthChoice Illinois, and MMAI plans.
