Summary
Blue Cross Blue Shield of Michigan has introduced two new Star measures for Medicare Advantage plans focused on medication safety: Poly-ACH (monitoring multiple anticholinergic medications in seniors) and COB (concurrent opioid and benzodiazepine use). Immediately: All providers must evaluate medication regimens at each office visit for Medicare beneficiaries 65+ to identify and document: 1) Multiple anticholinergic medications used for 30+ days, 2) Concurrent opioid and benzodiazepine use for 30+ days. Document evaluation of adverse effects, medical necessity assessments, and any medication changes or tapering decisions. Update clinical workflows to include medication safety checks.
