Summary
Long Term Care Nursing Facilities must submit facility payee information and W-9 forms to receive Quality Incentive and CNA Wage Pass-Through payments from MCO plans. MCO plans cannot distribute these payments without this required payee information.
Action Required
By August 10, 2022: Long Term Care Nursing Facilities must complete and submit the Directed Payment Data Collection Form (Excel or PDF) and current W-9 to HFS.LTC@illinois.gov with form name in email subject line. Include facility, contact, and banking information for MCO payment processing. Without this submission, facilities will not receive Quality Incentive or CNA Wage Pass-Through payments from MCO plans.
