Summary
Illinois requires all Medicaid managed care providers to use a standardized roster template when adding or updating provider information in network listings. Current providers must submit monthly roster updates for any changes to provider status, names, locations, or new providers joining the practice.
Action Required
Monthly: Complete and submit the standardized provider roster to all HealthChoice Illinois MCOs for any provider changes including name changes, office location updates, or new providers joining the practice.
