Illinois hospital providers must switch from NCR/JetPay to ACH Direct Debit for assessment payments effective December 31, 2022. ACH forms must be submitted by January 1, 2023, with automatic debits starting 4 business days before due dates beginning with the January 26, 2023 assessment.

By January 1, 2023: Illinois hospital billing teams must complete and submit ACH Direct Debit forms to HFS.ProviderAssessmentUnit@illinois.gov to replace NCR/JetPay payment method. Download forms from hfs3848G.pdf on illinois.gov. Notify CFO and mailroom of incoming assessment notices. Failure to submit will prevent required assessment payments starting January 26, 2023.
