By January 26, 2023: Illinois hospital providers must ensure their bank accounts are set up to accept ACH debits from Wells Fargo (origination ID 4550895818, routing number 091000019). Contact your bank to add debit block filter 4550895818 if debit blocks are in place. If ACH forms are not completed, mail paper check with remittance card postmarked by January 26, 2023 to avoid penalties. Finance team should verify payment processing and contact HFS Provider Assessment Unit at 217-524-7110 with questions.
