Immediately: Long-term care billing teams should download the HFS 2449A LTC Transaction Report from the MEDI system to review updated patient credit amounts. Use the HFS payment system via MEDI LTC Inquiry as the authoritative source for patient liability amounts, not the 360 Notice of Decision. If patient credit amounts appear incorrect, submit an LTC Income Change transaction in MEDI with proper verification documents to the appropriate LTC MFO office.
