If banking information changed since January 2023, submit updated ACH form to HFS.ProviderAssessmentUnit@illinois.gov. Provide bank the company ID 4550895818 and ACH routing 091000019 if debit blocks are used. Ensure sufficient funds for monthly debits through June 2024.
