Summary

Illinois Department of Healthcare and Family Services has changed the payment method for Ground Emergency Medical Transportation (GEMT) supplemental payments. Providers must now use a new ACH Direct Debit process with form HFS3881G instead of the old HFS3848G form, and FY2023 payments are due by January 31, 2024.

By January 31, 2024: Transportation providers enrolled in Illinois GEMT program must complete new form HFS3881G and submit to HFS.GEMT@Illinois.gov to enroll in updated ACH Direct Debit process. Alternatively, providers may remit payment via check noting 'GEMT Payment' with invoice number to Illinois Department of Healthcare and Family Services, Bureau of Fiscal Operations - GEMT, PO Box 19491, Springfield, IL 62794-9491.
