Summary
Illinois publicly owned ambulance providers must submit new cost reports by October 1, 2024 to participate in the 2025 Ground Emergency Medical Transportation (GEMT) supplemental payment program. New participants also need signed Intergovernmental Agreements from authorized local government officers.
Action Required
By October 1, 2024: Illinois publicly owned ambulance providers must complete and submit cost report forms (as Excel and signed PDF) to HFS.GEMT@illinois.gov. First-time participants must also submit signed Intergovernmental Agreements from Mayor, Village/Town Administrator, or Fire Protection District head. Failure to submit will result in exclusion from 2025 supplemental payment program.
